Diastat Gel


Pronunciation: dye-AZ-e-pam
Generic Name: Diazepam
Brand Name: Examples include Diastat and Diastat AcuDial


Diastat Gel is used for:

Treating episodes of increased seizure activity (cluster seizures) in certain patients with epilepsy who already take other seizure medicines.


Diastat Gel is a benzodiazepine. It works in the brain to decrease seizures.


Do NOT use Diastat Gel if:


  • you are allergic to any ingredient in Diastat Gel

  • you have narrow-angle glaucoma, severe liver disease, or certain severe mental problems (eg, psychosis)

Contact your doctor or health care provider right away if any of these apply to you.



Before using Diastat Gel:


Some medical conditions may interact with Diastat Gel. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:


  • if you are pregnant, planning to become pregnant, or are breast-feeding

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines or other substances

  • if you have a history of liver or kidney problems, lung or breathing problems (eg, asthma, chronic obstructive pulmonary disease [COPD], pneumonia), myasthenia gravis, porphyria (a blood disease), or glaucoma, or you are at risk for narrow-angle glaucoma

  • if you have a history of depression, other mental or mood problems, suicidal thoughts or actions, or alcohol or other substance abuse or dependence

Some MEDICINES MAY INTERACT with Diastat Gel. Tell your health care provider if you are taking any other medicines, especially any of the following:


  • Antidepressants (eg, amitriptyline, fluoxetine), barbiturates (eg, phenobarbital), HIV protease inhibitors (eg, ritonavir), monoamine oxidase inhibitors (MAOIs) (eg, phenelzine), narcotic medicines (eg, morphine, hydrocodone), phenothiazines (eg, chlorpromazine), valproate, or valproic acid because they may increase the risk of Diastat Gel's side effects, such as severe drowsiness and confusion

This may not be a complete list of all interactions that may occur. Ask your health care provider if Diastat Gel may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.


How to use Diastat Gel:


Use Diastat Gel as directed by your doctor. Check the label on the medicine for exact dosing instructions.


  • An extra patient leaflet is available with Diastat Gel. Talk to your pharmacist if you have questions about this information.

  • A health care provider will teach your caregiver how to use Diastat Gel. Be sure your caregiver understands how to use Diastat Gel. Contact your health care provider if you or your caregiver have any questions about using Diastat Gel.

  • If you are using Diastat AcuDial, the pharmacist will set the dial to the appropriate dose before you receive Diastat Gel. Check the syringe as soon as you pick up Diastat Gel from the pharmacy. Make sure that you can see the dose in the display window and that the dose is correct. Make sure that you can see the green "ready" band on the syringe just below the cap. If you have questions about how to tell whether the dial is set to the correct dose or whether the syringe is ready to use, contact your doctor or pharmacist.

  • To use Diastat Gel, place the patient on their side where they cannot fall. Get the syringe. Push up with your thumb and remove the cap from the syringe. Be sure the seal pin is removed with the cap. Lubricate the rectal tip with the lubricating jelly provided.

  • Turn the patient on their side facing you. Bend the upper leg forward and separate the buttocks to expose the rectum.

  • Gently insert the syringe tip into the rectum. Slowly count to 3 while gently pushing the plunger in until it stops. Slowly count to 3 before removing the syringe from the rectum. Slowly count to 3 while holding the buttocks together to prevent leakage of the medicine.

  • Keep the patient on their side facing you. Continue to observe the patient as directed by the patient's doctor.

  • Do not reuse the syringe or other materials. If you are using Diastat AcuDial, pull on the plunger until it is completely removed from the syringe. Point the tip of the syringe over the toilet or sink. Replace the plunger into the syringe, gently pushing it in until it stops. Flush the toilet or rinse the sink with water until you can no longer see any gel.

  • Throw away all used materials in the trash can. Keep out of the reach of children and away from pets.

  • Contact the patient's doctor at once if seizures continue for 15 minutes after using Diastat Gel, unless the patient's doctor tells you otherwise.

  • Contact the doctor at once if you are alarmed by the frequency or severity of the seizures, or the color or breathing of the patient.

  • Contact the doctor at once if the seizures are different from the patient's usual seizure episodes or if the patient is having unusual or serious problems.

  • Do NOT use Diastat Gel more often than once every 5 days unless your doctor tells you otherwise.

  • Do NOT use Diastat Gel more often than 5 times per month unless your doctor tells you otherwise.

  • If you miss a dose of Diastat Gel, contact your doctor right away.

Ask your health care provider any questions you may have about how to use Diastat Gel.



Important safety information:


  • Diastat Gel may cause drowsiness or dizziness. These effects may be worse if you take it with alcohol or certain medicines. Use Diastat Gel with caution. Do not drive or perform other possibly unsafe tasks until you know how you react to it.

  • Check with your doctor before you drink alcohol or use medicines that may cause drowsiness (eg, sleep aids, muscle relaxers) while you are using Diastat Gel; it may add to their effects. Ask your pharmacist if you have questions about which medicines may cause drowsiness.

  • Do NOT use more than the recommended dose or use more often than prescribed without checking with your doctor.

  • Contact the doctor at once if the patient develops new or worsening seizures.

  • Tell your doctor or dentist that you take Diastat Gel before you receive any medical or dental care, emergency care, or surgery.

  • Use Diastat Gel with caution in the ELDERLY; they may be more sensitive to its effects, especially drowsiness or decreased coordination.

  • Diastat Gel should be used with extreme caution in CHILDREN younger than 2 years old; safety and effectiveness in these children have not been confirmed.

  • Diastat Gel should not be used in CHILDREN younger than 6 months old; safety and effectiveness in these children have not been confirmed.

  • PREGNANCY and BREAST-FEEDING: Diastat Gel may cause harm to the fetus. If you think you may be pregnant, contact your doctor. You will need to discuss the benefits and risks of using Diastat Gel while you are pregnant. Diastat Gel is found in breast milk. Do not breast-feed while using Diastat Gel. Discuss with your doctor how long you should wait to breast-feed after using Diastat Gel.

Some people who use Diastat Gel for a long time may develop a need to continue taking it. This is known as DEPENDENCE or addiction. If you are addicted to Diastat Gel and you suddenly stop using it, you may have WITHDRAWAL symptoms. These may include feeling weak, dizzy, or lightheaded. You may also have nausea, muscle twitching, trembling of the hands and fingers, hallucinations, trouble sleeping, increased sweating, or seizures.



Possible side effects of Diastat Gel:


All medicines may cause side effects, but many people have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:



Decreased coordination; diarrhea; dizziness; drowsiness; headache; nervousness; stomach pain; stuffy nose.



Seek medical attention right away if any of these SEVERE side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); anxiety; hallucinations; mental or mood changes; muscle spasms or twitching; new or worsened seizures; overexcitement; shortness of breath; sleep disturbances; trouble sleeping; wheezing.

If OVERDOSE is suspected:


Contact 1-800-222-1222 (the American Association of Poison Control Centers), your local poison control center, or emergency room immediately. Symptoms may include coma; confusion; loss of consciousness; severe drowsiness; slow reflexes.


Proper storage of Diastat Gel:

Store Diastat Gel at 77 degrees F (25 degrees C). Brief storage at temperatures between 59 and 86 degrees F (15 and 30 degrees C) is permitted. Do not store in the bathroom. Keep Diastat Gel, as well as syringes and needles, out of the reach of children and away from pets.


General information:


  • If you have any questions about Diastat Gel, please talk with your doctor, pharmacist, or other health care provider.

  • Diastat Gel is to be used only by the patient for whom it is prescribed. Do not share it with other people.

  • If your symptoms do not improve or if they become worse, check with your doctor.

  • Check with your pharmacist about how to dispose of unused medicine.

Generic Name: sevelamer (se VEL a mer)

Brand Names: Renagel, Renvela


What is sevelamer?

Sevelamer is a phosphate binder. Sevelamer helps prevent hypocalcemia (low levels of calcium in the body) caused by elevated phosphorus.


Sevelamer is used to reduce blood levels of phosphorus in people with chronic kidney disease who are on dialysis.


Sevelamer may also be used for purposes not listed in this medication guide.


What is the most important information I should know about sevelamer?


You should not take this medication if you are allergic to sevelamer, or if you have a bowel obstruction.

Before taking sevelamer, tell your doctor if you have trouble swallowing, severe constipation, a blockage in your intestines, a stomach or intestinal disorder, or if you have recently had stomach or intestinal surgery.


Avoid taking any other medicines within 1 hour before or 3 hours after you take sevelamer. Sevelamer can bind to other medications and make them less effective.


Before taking sevelamer, tell your doctor if you are taking ciprofloxacin (Cipro), a heart rhythm medication, or a seizure medication.


Do not take calcium or other mineral supplements without your doctor's advice. Use only the specific type of calcium or mineral supplement your doctor recommends.

What should I discuss with my healthcare provider before taking sevelamer?


You should not take this medication if you are allergic to sevelamer, or if you have a bowel obstruction.

To make sure you can safely take sevelamer, tell your doctor if you have any of these other conditions:



  • trouble swallowing;




  • severe constipation;




  • a blockage in your intestines;




  • a stomach or intestinal disorder; or




  • if you have recently had stomach or intestinal surgery.




FDA pregnancy category C. It is not known whether sevelamer will harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant while using this medication. It is not known whether sevelamer passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

How should I take sevelamer?


Take exactly as prescribed by your doctor. Do not take in larger or smaller amounts or for longer than recommended. Follow the directions on your prescription label.


Your doctor may occasionally change your dose to make sure you get the best results.


Take sevelamer with meals. Do not crush, chew, or break the sevelamer tablet. Swallow the pill whole. Sevelamer tablets expand when they are wet, and breaking or crushing the pill may make it harder to swallow.

Sevelamer powder must be dissolved in water before you take it. The 0.8-gram packet should be mixed with at least 1 ounce (2 tablespoons) of water. The 2.4-gram packet should be mixed with at least 2 ounces (4 tablespoons) of water.


Stir the powder in water until it is completely dissolved. Drink all of this mixture right away. To make sure you get the entire dose, add a little more water to the same glass, swirl gently and drink right away.


To be sure this medication is helping your condition and not causing harmful effects, your blood may need to be tested on a regular basis. Do not miss any scheduled appointments.


Sevelamer may be only part of a complete program of treatment that also includes a special diet. It is very important to follow the diet plan created for you by your doctor or nutrition counselor. You should become very familiar with the list of foods you must eat or avoid to help control your condition.


Store at room temperature away from moisture and heat.

What happens if I miss a dose?


Take the missed dose as soon as you remember. Be sure to take the missed dose with food. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.


What happens if I overdose?


Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

What should I avoid while taking sevelamer?


Avoid taking any other medicines within 1 hour before or 3 hours after you take sevelamer. Sevelamer can bind to other medications and make them less effective.


Do not take calcium or other mineral supplements without your doctor's advice. Use only the specific type of calcium or mineral supplement your doctor recommends.

Sevelamer side effects


Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop using sevelamer and call your doctor at once if you have a serious side effect such as:

  • black, bloody, or tarry stools;




  • constipation that gets worse or does not clear up;




  • severe constipation with stomach pain; or




  • fever, chills, body aches, flu symptoms.



Less serious side effects may include:



  • nausea, vomiting, stomach pain, loss of appetite;




  • upset stomach, gas, bloating;




  • diarrhea, mild constipation;




  • tired feeling;




  • itching; or




  • joint pain.

What other drugs will affect sevelamer?


Tell your doctor about all other medicines you use, especially:



  • ciprofloxacin (Cipro);




  • levothyroxine (Synthroid, Levothroid, Levoxyl);




  • a heart rhythm medication; or




  • a seizure medication.



This list is not complete and other drugs may interact with sevelamer. Tell your doctor about all medications you use. This includes prescription, over-the-counter, vitamin, and herbal products. Do not start a new medication without telling your doctor.

Os-Cal 500+D


Pronunciation: KAL-see-um KAR-bo-nate with VYE-ta-min D
Generic Name: Calcium Carbonate with Vitamin D
Brand Name: Examples include Caltrate 600+D and Os-Cal 500+D


Os-Cal 500+D is used for:

Treating or preventing calcium deficiency. It may also be used for other conditions as determined by your doctor.


Os-Cal 500+D is a dietary supplement. It works by providing extra calcium to the body.


Do NOT use Os-Cal 500+D if:


  • you are allergic to any ingredient in Os-Cal 500+D

  • you have high blood calcium levels or high blood vitamin D levels

Contact your doctor or health care provider right away if any of these apply to you.



Before using Os-Cal 500+D:


Some medical conditions may interact with Os-Cal 500+D. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:


  • if you are pregnant, planning to become pregnant, or are breast-feeding

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines, foods, or other substances (including shellfish)

  • if you have high blood phosphate levels or high levels of calcium in the urine

  • if you have dehydration, heart problems, hardening of the arteries, kidney problems, kidney stones, or sarcoidosis

Some MEDICINES MAY INTERACT with Os-Cal 500+D. Tell your health care provider if you are taking any other medicines, especially any of the following:


  • Mycophenolate because its effectiveness may be decreased by Os-Cal 500+D

How to use Os-Cal 500+D:


Use Os-Cal 500+D as directed by your doctor. Check the label on the medicine for exact dosing instructions.


  • Take Os-Cal 500+D by mouth with or without food.

  • Take Os-Cal 500+D with a full glass of water (8 oz/240 mL).

  • If you also take azole antifungals (eg, ketoconazole), bisphosphonates (eg, etidronate), cation exchange resins (eg, sodium polystyrene sulfonate), cephalosporins (eg, cefdinir), doxycycline, iron, quinolones (eg, ciprofloxacin, levofloxacin), tetracyclines (eg, doxycycline), or thyroid hormones (eg, levothyroxine), ask your doctor or pharmacist how to take them with Os-Cal 500+D.

  • If you miss a dose of Os-Cal 500+D, take it as soon as you remember. Continue to take it as directed by your doctor or on the package label.

Ask your health care provider any questions you may have about how to use Os-Cal 500+D.



Important safety information:


  • Do not take large doses of vitamins while you use Os-Cal 500+D unless your doctor tells you to.

  • Tell your doctor or dentist that you take Os-Cal 500+D before you receive any medical or dental care, emergency care, or surgery.

  • This product may contain tartrazine dye (FD&C Yellow No. 5). This may cause an allergic reaction in some patients. If you have ever had an allergic reaction to tartrazine, ask your pharmacist if your product has tartrazine in it.

  • Lab tests, including serum calcium levels, may be performed while you use Os-Cal 500+D. These tests may be used to monitor your condition or check for side effects. Be sure to keep all doctor and lab appointments.

  • Os-Cal 500+D should be used with extreme caution in CHILDREN; safety and effectiveness in children have not been confirmed.

  • PREGNANCY and BREAST-FEEDING: If you become pregnant, contact your doctor. You will need to discuss the benefits and risks of using Os-Cal 500+D while you are pregnant. It is not known if Os-Cal 500+D is found in breast milk. If you are or will be breast-feeding while you use Os-Cal 500+D, check with your doctor. Discuss any possible risks to your baby.


Possible side effects of Os-Cal 500+D:


All medicines may cause side effects, but many people have no, or minor, side effects. No COMMON side effects have been reported with Os-Cal 500+D. Seek medical attention right away if any of these SEVERE side effects occur:



Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); loss of appetite; nausea; severe or persistent constipation; vomiting.

Contact 1-800-222-1222 (the American Association of Poison Control Centers), your local poison control center, or emergency room immediately. Symptoms may include confusion; delirium; loss of consciousness; mood or mental changes; sluggishness.


Proper storage of Os-Cal 500+D:

Store Os-Cal 500+D at room temperature, between 59 and 86 degrees F (15 and 30 degrees C). Store away from heat, moisture, and light. Do not store in the bathroom. Keep Os-Cal 500+D out of the reach of children and away from pets.
